Janitor

In the series' pilot episode, protagonist J.D. sees Janitor fixing a sliding door that was stuck, and suggests someone might have stuck a penny in the door. Janitor tells J.D., "If I find a penny in there, I'm taking you down." He has spent the rest of the series tormenting J.D. Some of his practical jokes have been on the severe side, such as destroying J.D.'s bike, trapping him in a water tower, and stranding him in the middle of nowhere.
Neil Flynn, who plays Janitor, has said: "I think it's possible that he doesn't hate J.D. Maybe J.D. is as close to a friend as he has. For all we know, he just has poor social skills. This may have been shown when The Janitor decides to torment Turk and calls him 'new friend'. I think that the Janitor constantly misreads J.D.'s motives and assumes he's a rich young punk."It is suggested that the Janitor has a troubled past. During Carla's baby shower, he presents her with a "baby cage," which he says is nicer than the one he had as a child

Carla Espinosa Turk

Carla is the head nurse at Sacred Heart Hospital, in which the main action of the show takes place.
She is also one of the most mature characters in Scrubs, and acts like a mother to many of the characters, particularly J.D.
Carla originates from the Dominican Republic, and is fluent in Spanish. She speaks in Spanish to her brother and her mother, and we first find that she is a Spanish speaker in "My Old Lady", when she translates for Elliot and her patient. She also regularly gives insults in Spanish, and in "My Musical", she gives the Todd ways to say sexual innuendo in Spanish.
She is prone to overreaction and constantly feels the need to tell friends and co-workers how to go about their lives (most notably the series main character J.D.) and this advice is often unwanted, if not simply intrusive. She is also obsessed with spreading gossip, and when she swore not to spread a secret about Elliot Reid, she walks through the hospital reception awkwardly avoiding answering questions; when Janitor asks of Elliot's whereabouts, she replies: "cheese". She calls bossing people around "her crack" in the fifth season. Despite her personal foibles, no one dares make fun of her; when anyone does, she humiliates them in front of the entire hospital; this regime is briefly altered in one episode, where in a failed attempt to retain her dignity Carla says that she allows her friends to make fun of her. After several jokes about her, she loses her temper with Doug Murphy.

Christopher Duncan Turk aka Turk

Christopher Turk, commonly referred to by his surname, is the best friend and former roommate of the series' protagonist, J.D. He is currently an attending surgeon at Sacred Heart Hospital, where the series takes place, and is married to Carla Espinosa, one of the nurses. He was most likely born somewhere around 1976, as he first left for college in 1993/1994, during which time, as well as completing his pre-med requirement, he completed a minor in Psychology. In "My Bad", he reveals that he regularly attended therapy during this time in order to deal with the stress.
Turk is a very competitive person. He says that his competitive nature drove him to "play everything" in high school, and he also cites it as the main reason why he became a surgeon. He can frequently be seen throughout the series playing basketball outside the hospital, which helps him to relax before he has to perform surgery, or engaging J.D. in childishly competitive games. J.D often doesn't really want to participate in the games Turk challenges him to. Turk is normally the victor of these games, and he enjoys rubbing it in J.D.'s face. He normally does this by making J.D. say "I'm your biatch" after J.D. loses.
In "My Cake" Turk is diagnosed with type two diabetes; he at first takes it as a joke, but now takes it seriously.
Turk is Roman Catholic, though his faith is tested in the episode "My Own Personal Jesus" by numerous casualties occurring on Christmas Eve. His faith is restored at the end of the episode by his miraculous finding of a missing pregnant girl in severe need of medical attention.
As evidenced by "His Story IV", Turk appears to be a political liberal; he votes Democrat and does not support the war in Iraq. He is also frequently seen wearing Amnesty International t-shirts. He states in a deleted scene of "My Way Home" that he is a Democrat.Along with being a gifted singer, Turk is an extremely talented dancer. In "My Half-Acre", he becomes a member of the Cool Cats, the hospital air band put together by the Janitor, Ted Buckland, and Lloyd, the hospital's delivery man. He also seems to be very attracted to Tyra Banks, admitting he has a picture of her in his cell phone.

John Michael Dorian aka JD

J.D. is the younger son of Sam and Barbara Dorian. He has one older brother, Dan, played by Tom Cavanagh. In "My Office", it was revealed that J.D. had grown up in Trotwood, Ohio. John Ritter played his father, who died in "My Cake" as a tribute to Ritter's death. Sam was an office supplies salesman, J.D. describing him as "a bad one." He did provide for the family, giving J.D. one bike part per year as a birthday present. Sam and Barbara got divorced when J.D. was seven, so Sam left home. Barbara then got remarried six times. She lives with her new fiancée, who kicked Dan out.
In "My Own Private Practice Guy", J.D. stated that he had a grandmother who was still alive, although he claimed that he would feel awful if she did die when he lied about her death as an excuse. His grandmother also appears in a flashback in "My Bed Banter & Beyond". As of "My Advice to You", Grandma Dorian had died. In the same episode, J.D. also mentioned Nana Hobbs. He claimed that she was "a teensy bit racist", a claim backed up by J.D.'s best friend, Turk, explaining that she attacked him believing he was robbing her house.

In "My Quarantine", he says that he had an Uncle Carleton who had red eyes because he was allergic to preserves but still spread them on everything.
Currently, his now "off-again" girlfriend Dr. Kim Briggs gave birth to his son, who they have thus far decided will be named Sam Perry Gilligan Dorian. Sam Dorian was born at the end of "My Hard Labor". The middle name Perry is in honor of Dr. Perry Cox. The Gilligan is because he lost a bet with Turk.J.D.'s only pet, shared with Turk, is a stuffed Yellow Labrador named Rowdy in his apartment, whom Turk bought at a garage sale. In season four, he was replaced by a similar (albeit slightly shorter) stuffed dog named Steven. Rowdy was later returned, as it turned out the Janitor had found and kept Rowdy for two years, until the time he needed it to barter with Carla. J.D. would keep Steven, and Turk would keep Rowdy
